COVID-19 WAIVER/RELEASE FOR USE OF TOWN POOL

The undersigned hereby represents, confirms, and knowingly acknowledges the following:

1. I am over the age of 18.

2. I am in good health, with no symptoms of COVID-19 and no medical conditions that place me at increased risk for severe illness from COVID-19. If I develop symptoms of or am exposed to someone having symptoms of COVID-19, I will not use the Town of North Haven Pool. I understand that people living in households with individuals who are 65 years and older OR have higher risk for severe illness from COVID-19 are recommended to stay home.

3. I have reviewed the State of Connecticut Sector Rules for June 17^th Reopen, specifically relating to pools/pool activities (https://portal.ct.gov/-/media/DECD/Covid_Business_Recovery-Phase-2/Sports_FitnessCenters-_C4_V1.pdf) and agree to comply with, follow and adhere to the Rules, as well as follow, comply with and adhere to the Town of North Haven’s rules and requirements for use of the swimming pool, at all times.

4. All minors (children under the age of 18) shall be constantly supervised by their parent or guardian and by signing below, I am agreeing to the terms of this waiver/release on behalf of any minor accompanying me to the Town Pool.

5. In using and accessing the Town’s property, I, along with my guests and invitees, shall comply with all applicable statutes, laws, regulations, codes, rules or orders of or issued by any governmental body having jurisdiction over the use of the Town Pool, including but not limited to, any Executive Orders issued by Governor Ned Lamont and any Sector Rules adopted and implemented by the State of Connecticut requiring cleaning and/or social distancing practices to combat the spread and infection of COVID-19.

6. I acknowledge that an inherent risk of exposure to COVID-19 exists in any public place where people are present. By using the Town Pool, I and any guests/invitees accompanying me voluntarily assume all risks related to exposure to COVID-19 and expressly, knowingly and voluntarily agree not to hold the Town of North Haven, its employees, elected and appointed boards and commissions, or its elected and appointed officials liable for any illness or injury.

7. I acknowledge that the Town reserves the right to revoke permission for my use of the Town Pool, without notice, should it observe non-compliance with applicable rules or if it deems revocation to be in the best interest of the Town.

I have read, understand, and agree to the terms and conditions of this waiver/release. This waiver/release is effective on the date below and remains in full force and effect until revoked or cancelled by the Town.
